One person supported by Active Pathways has recently moved from The Hamptons, our inpatient service in Preston, to Brookhaven in Bamber Bridge, which provides residential care with mental health nursing.
The move followed a period of good progress at The Hamptons. When the person was ready for a different level of support, colleagues from both services worked together to plan the transition and share their knowledge of the person’s needs, strengths, goals and identified risks.
Moving to Brookhaven meant they could continue their progress in a less restrictive setting, while still receiving the support they needed.
Continuing to build independence at Brookhaven
Since moving to Brookhaven, the person has settled well and taken greater responsibility for their day-to-day life. They manage their daily routine, personal responsibilities and schedule independently, while previously identified risks remain low.
They have also taken on the role of ‘Voice of the People’ at Brookhaven. They gather feedback from others living at the service and contribute to decisions about activities, events and menus.
This gives people at Brookhaven a greater say in daily life at the service, including those who may be less confident about sharing their views. It has also enabled the person to put their own confidence and experience to good use by representing others.
Our care pathway in practice
This move shows how our care pathway can support people as their needs change.
Active Pathways provides inpatient care, residential care with mental health nursing, residential care and enhanced supported living. People can move between different levels of support, with colleagues and our in-house therapeutic teams working together to maintain consistency and build on the progress already made.
For this person, moving from The Hamptons to Brookhaven was an important next step. Their progress can also be seen in the things they now do each day: managing their own routine, taking responsibility and making a positive contribution to life at Brookhaven.
