Enhanced Supported Living with Active Pathways

Our enhanced supported living services help adults to live more independently  – with access to the right support, if they need it. The services are carefully designed to offer a stable environment that balances safety and support with autonomy, so each person can live a meaningful life in the community.

People we support

We support adults aged 18 and over who are ready for more independent living in the community.

We work alongside our clinical and therapeutic teams to ensure the service is right for each person, with clear risk management and progression plans in place.

Our supported living services include:

  • Individual en-suite bedrooms or self-contained apartments with a kitchen, living area and bathroom
  • Shared spaces for therapeutic engagement and social interaction
  • Thoughtfully designed environments that promote autonomy and safety
  • On-site staff support 24/7

Specialist input from our wider therapeutic team and clinicians

Where appropriate and agreed with the funding authority, residents may benefit from coordinated support through Active Pathways’ multidisciplinary expertise:

  • Occupational therapy assessments and skill-building programmes
  • Psychological input where appropriate, including behaviour support
  • Clear links with external clinical services, including community mental health teams and GPs

Our staff teams receive regular training in positive behaviour support, trauma-informed care, and risk planning.

A focus on safety and sustainability

A shared risk protocol is developed in collaboration with the resident, family, professionals and staff. 

This includes:

  • Clear guidance on how to respond to risks or deterioration in mental health
  • A consistent, respectful approach to boundaries and behaviours
  • Any plans can adapt as people grow in confidence and ability to live independently
