Residential care at Active Pathways

Our residential care services provide therapeutic support for adults with complex mental health needs who are stepping down from hospital or require longer-term support. We offer safe, structured environments where residents can regain confidence, build skills, and move towards greater independence.

Our services support men and women aged 18 and over who:

  • Diagnosed with mental health conditions, sensory processing differences, autism, with or without a learning disability
  • May have a forensic history or have experienced placement breakdowns
  • Are stepping down from hospital, or require longer-term support in a safe, structured setting

We work with people under Community Treatment Orders, Deprivation of Liberty Safeguards, or informal arrangements.

Recovery-focused, person-led support

Every resident is supported to rebuild daily living skills and plan for the future at their own pace.

Our teams focus on:

  • Strengths-based, personalised care planning
  • Emotional wellbeing, resilience and relapse prevention
  • Life skills, relationships and routine building
  • Confidence in managing risks and making decisions
    Encouraging meaningful activity and community access
  • Support is co-produced with residents and adapted to their changing needs and goals.

Psychology

Our psychology team provides clinical input and guidance tailored to each individual.

Interventions may include, but are not limited to: 

  • Cognitive assessments and risk formulation
  • Individual and group therapy sessions
  • Evidence-based models including:
  • Dialectical Behavioural Therapy (DBT)
  • Cognitive Behavioural Therapy (CBT)
  • Acceptance and Commitment Therapy (ACT)
  • Mindfulness
  • Positive Behaviour Support (PBS)Residents are encouraged to explore what works best for them, with support from psychologists and the wider team.

Occupational Therapy

OTs help residents regain structure and meaning in daily life.

Support is personalised and may include:

  • Assessments to understand strengths and areas of need goal setting and personalised OT plans
  • Support with routines, self-care, budgeting and skills for independence
  • Skill-building in areas like cooking, personal care, and travel
  • Group and 1:1 sessions focused on recovery through activity

Recovery Support Workers

Our support staff build strong, trusting relationships and work side-by-side with residents to:

  • Manage emotions and stay engaged in recovery
  • Build confidence through daily routines and positive risk-taking
  • Participate in activities and access the local community
  • Plan next steps towards greater independence

    Building everyday skills and self-reliance

    Residents are supported to:

      • Manage personal care, medication and routines

      • Develop cooking, cleaning and organisational skills

      • Build social and communication confidence

      • Travel safely and manage appointments

      • Plan for volunteering, education or employment

    Our focus is always on recovery, progress and future planning. Residents are supported to move forward in a way that is safe for them and others. We see the potential in every person we support.
