At Active Pathways, we believe everyone deserves a life filled with meaning, dignity and purpose. For over 20 years, we have supported people with complex mental health conditions, autism spectrum disorders, learning disabilities, and other complex needs. Our person-centred approach enables individuals to grow, thrive, and progress at their own pace, in therapeutic settings tailored to their unique needs.

Our services offer a clear, consistent step-up, step-down pathway – from inpatient hospital care through to residential living and enhanced supported living in the community.

We provide the right support at the right time, with a focus on building confidence, personal skills and long-term stability.

Our story

Founded with a vision to create safe, innovative yet personalised alternatives to hospital-based care, Active Pathways has grown to support people across the North of England through a blend of therapeutic, residential and enhanced supported living services.

We partner with local authorities and Integrated Care Boards, with a reputation for:

  • Responding quickly to urgent or complex referrals
  • Adapting and reconfiguring services to meet evolving needs
  • Delivering excellent clinical outcomes and value for money

Our approach to care

We are proud of the strong therapeutic and clinical framework that underpins every service we deliver. Each of our residential and supported living settings includes a full in-house therapeutic team, working closely with individuals, families and commissioners to support recovery and sustained progress.

Our services include:

  • Psychology-led care planning and therapeutic input
  • Occupational therapy assessments and group sessions
  • Behavioural support and risk management
  • Drug and alcohol support embedded in care plans, if required 
  • Daily living skills and community integration support
  • Mental health nursing in our service in Brookhaven, Preston
