Brook House is a purpose-built supported living development comprising of 13, one-bedroomed apartments. Each apartment comes beautifully decorated, including white goods and ready to be furnished.

All apartments are decorated in beautiful neutral tones, allowing each resident to personalise their apartment to create a space that reflects their character, and make it truly feel like home.

Our staff apartment and communal space ensures residents of Brook House have access to support 24 hours a day, whether that’s questions regarding welfare, support needs, or just want a chat with a friendly face, there is always someone familiar around that forms part of our incredible Active Pathways team.

Our Model  

Individuals are at the heart of our person-centred supported living model. Support is offered to people with needs around their mental health, learning disability and/or Autism.

We will endeavour to provide the impetus that supports and builds on each person’s aspirations and objectives.  We focus on supporting everyone to achieve the best they can by empowering them to build relationships, networks and maintain their well-being. We recognise that everyone is unique and different, that they have their own individual set of needs which we will support throughout their pathway to a brighter future. We aim to do this by providing the stabilising and enabling support that promotes independence whilst remaining healthy and safe. 

What do we offer? 

Staff are available 24 hours a day to assist individuals identified as requiring additional support. This can range from a number of weekly hours to assist with household chores, budgeting and medication, to a more comprehensive package of care supporting personal care needs.

We offer a core support package that includes background staff 24 hours a day, as well a sleep-in staff member overnight. From there we can build a bespoke support package, that is tailor-made to include 1-1 support, Psychology and Occupational Therapy. This is created in conjunction with the person, their families, our multi-disciplinary team and other key stakeholders.

Our aim is to create a home environment where a person feels safe and receives quality support delivered by a passionate team.
