As a Support Worker, you will:
- Support individuals on their journey to recovery through person-centred care in a rehabilitation setting.
- Help residents develop independent living skills and achieve personal goals.
- Work within a multidisciplinary team to build positive relationships with residents.
- Provide emotional and practical support to residents.
- Promote engagement in meaningful activities and encourage social inclusion.
- Assist with daily living tasks and support residents in accessing education, employment, and the community.
- Promote independence and self-confidence, helping residents regain control over their lives.

Active Pathways is currently seeking dedicated Bank Support Workers to work across Park Place (Supported Living Service) and Millcroft (Step-Down Residential Rehabilitation Service) in Seacroft, Leeds. This is a fantastic opportunity for a passionate individual who wants to make a real difference in the lives of the residents within their care.
Key Responsibilities
- Support service users in achieving their life goals, promoting independence and enhancing their quality of life.
- Develop, implement, and maintain person-centered support plans tailored to individual needs.
- Handle challenging behavior with professionalism, demonstrating resilience and problem-solving skills.
- Administer medication in accordance with guidelines, ensuring compliance and safety.
- Provide care and support during lone working shifts, ensuring the safety and well-being of the residents.
- Build and sustain trusting relationships, offering guidance and support to help residents lead full, happy, and healthy lives.
Experience & Skills
- Previous experience within a Mental Health or Rehabilitation setting.
- Familiarity with support plans and medication administration.
- Strong understanding of challenging behaviors and how to handle them effectively.
- Ability to work independently, showing resilience and initiative, especially in lone working scenarios.
- A collaborative approach to working within a multidisciplinary team.
- Committed to delivering person-centered, high-quality care.
